Academic Advising Forms
Technology Division Advising has several forms that can be used to change your academic information. Some of them are paper forms that must be filled out and submitted to your assigned advisor. Please contact your advisor if you have any questions about what forms are necessary for your academic plans.
For an in-progress grade report, please print and return a filled out paper copy of the in-progress grade report form to your assigned advisor.
There is also an online form for a change of major and an information request form for any type of general information that you may need from Technology Division Advising.
